What Is Project REHABILITATE?
Project REHABILITATE is a comprehensive juvenile‑justice intervention and reentry initiative designed to help justice‑involved youth heal from trauma, develop positive behavioral skills, strengthen family relationships, and successfully reintegrate into their schools and communities.
Through an integrated continuum of behavioral‑health, educational, therapeutic, family‑centered, and reentry services, Project REHABILITATE:
addresses underlying factors contributing to delinquency while building the protective factors youth need for long‑term stability, accountability, and success.
promotes emotional wellness, personal responsibility, family engagement, community connectedness, and positive youth development, grounded in evidence‑informed and trauma‑responsive practices.
helps youth reduce recidivism, strengthen emotional regulation, improve decision‑making skills, and build pathways toward healthy, productive futures through individualized support and coordinated interventions.
Project REHABILITATE consists of eight core services that work together to support rehabilitation, resilience, and successful community reintegration.
Behavioral Health Case Management
Behavioral Health Case Management provides intensive, trauma-informed support that helps youth and families navigate the challenges associated with justice involvement, reentry, and behavioral health needs. Through individualized planning, ongoing guidance, service coordination, and family engagement, Case Managers help stabilize youth, strengthen emotional wellness, monitor progress, and connect participants to critical community resources. This intervention serves as the foundation of care, ensuring youth receive consistent support while working toward educational, behavioral, and personal goals.
Correctional Education and Court-Ordered Programs
Correctional Education and Court-Ordered Programs provide structured educational and rehabilitative services that promote accountability, self-awareness, and positive behavior change while helping youth fulfill juvenile court requirements. Youth participate in evidence-informed courses covering topics such as anger management, gang awareness, substance use prevention, victim empathy, healthy relationships, parenting skills, and personal responsibility. This intervention helps youth develop healthier decision-making skills, understand the impact of their actions, complete court-ordered requirements, and demonstrate progress toward rehabilitation and successful community reintegration.
Cognitive Behavioral Interventions
Cognitive Behavioral Interventions are delivered through the Well-Being Promotion Program (WBPP), which helps youth strengthen emotional resilience, improve problem-solving abilities, and develop healthier patterns of thinking and behavior. Through guided reflection, skill-building activities, and strengths-based practices, participants learn to manage stress, cope with trauma, and understand the connection between thoughts, emotions, and actions. This intervention helps youth build confidence, increase self-efficacy, and develop the tools necessary for long-term personal growth and success.
Mentorship
Mentorship connects youth with trusted, caring adults who provide encouragement, guidance, accountability, and consistent support throughout the rehabilitation and reentry process. Mentors help participants strengthen self-confidence, improve decision-making skills, and build positive relationships while serving as role models for healthy behaviors and responsible choices. Through these supportive relationships, youth develop a stronger sense of belonging, purpose, and motivation to achieve their goals.
Counseling
Counseling provides individualized therapeutic support that promotes healing, emotional wellness, and personal growth. Within a safe and supportive environment, youth address trauma, manage stress, strengthen emotional regulation, and develop healthy coping strategies. Counseling also supports substance use recovery and other behavioral health needs, helping youth build the emotional stability and resilience necessary to navigate challenges and thrive.
Reentry Planning
Reentry Planning prepares youth for a successful transition from juvenile justice involvement back into their homes, schools, workplaces, and communities. Through individualized planning, resource connection, and skill development, youth identify goals, address barriers, and establish the supports necessary to maintain stability and avoid future system involvement. This intervention promotes self-sufficiency, strengthens protective factors, and creates a pathway toward long-term success.
Parent and Caregiver Education
Parent and Caregiver Education strengthens the family system by providing caregivers with the knowledge, skills, and trauma-responsive strategies needed to support their adolescent’s rehabilitation and development. Participants learn effective communication techniques, emotional regulation strategies, behavior management practices, and system-navigation skills that strengthen family functioning and stability. By enhancing caregiver capacity and fostering healthy family relationships, this intervention creates a supportive home environment that reinforces positive change and long-term success.
Caregiver and Parent Safety Planning
Caregiver and Parent Safety Planning helps families develop structured, trauma-responsive plans that promote safety, stability, and effective crisis response. Through collaborative planning, caregivers learn to recognize warning signs, implement coping strategies, access supportive resources, and maintain a safe and structured home environment. These plans strengthen family preparedness, increase protective factors, and demonstrate a proactive commitment to supporting youth well-being, successful reunification, and long-term family stability.
